Frequently Asked Questions

Question1:

Do you take my insurance?

I will give you a receipt/superbill you can submit for reimbursement from private insurance companies.

If your primary insurance is Medicare and wish to be treated by me there is a form to fill out to acknowledge that I am opt-ed out of Medicare and the superbill will not be sent to Medicare for reimbursement. (You can send it to your secondary non-Medicare insurance.)
